The screw securing the clicker toy to the Entertainer's tray can come out, presenting a choking hazard to young children. CPSC and Graco have received eight reports of children rotating the toy until the screw came out, and either handling the toy's screw or placing it in their mouths. One child started choking on the screw and sustained scratches in the throat, and two others sustained scratches on the face or stomach.
